heading-style

Formatter: writes a heading underlined with === or --- with # instead, on one line.

Example#

doc.md:

Install
=======

Run the installer.

markright check reports:

doc.md:1: heading underlined instead of written with `#` (heading-style)
doc.md:2: heading underlined instead of written with `#` (heading-style)
markright: 2 issue(s)

markright fmt rewrites it to:

# Install
